engine: mod_bmodel: refactor unified bmodel loader

* Use offsets into dbspmodel_t struct, get rid of srcmodel global variable.
* Allocate dbspmodel_t when it's required
* Make srclumps const, move Blue-Shift hack into lump loader's flag.
